I used that form to import my DLT-19. I filled in the form, but also included a photo of a Scout Trooper in armour holding the prop so they knew it was legit. I got the appropriate signed permit sent back to me fairly quickly. Once the blaster arrived in Australia I got a letter from Customs saying I had 30 days to present a permit, and the address to send it to. After that it sat in Customs for about a month and then finally got delivered.
Another option to build your own is buy a kit from Doopy-Doos. They're not super fast, and don't even try contacting their customer support, but the kits are pretty good and you probably won't need a permit to bring it in.
